David R. Gaugh, Vice Chair

David R. Gaugh, R.Ph., is the Vice President and General Manager of the Bedford Laboratories Division of Ben Venue Laboratories, a Boehringer Ingelheim Company, where he is responsible for Business Development, Marketing and Sales for the multi-source injectable business. Prior to his current position at Bedford, Mr. Gaugh served as the Director of Marketing and Business Development and General Manager and has held the position of Vice President and General Manager since January 2007.
Before joining Ben Venue, Mr. Gaugh was Senior Director, Pharmacy Contracting and Marketing at Novation, the largest Group Purchasing Organization in the United States. Prior to Novation, he was System Director of Pharmacy for St. Luke's Health System, a teaching hospital in Kansas City, Missouri.
Mr. Gaugh is a registered pharmacist and a graduate of the University of Wyoming School of Pharmacy. He is engaged in several pharmacy-related activities, such as the GPhA Bio-Similar Task Force, the ASHP Foundation Corporate Development Committee and various pharmacy/marketing internship programs. |

Henri R. Manasse, Jr., President

Henri R. Manasse, Jr., Ph.D., Sc.D., is executive vice president and chief executive officer of ASHP. The Executive Vice President of ASHP serves as the President of the Foundation Board and serves as the official liaison from ASHP to the Foundation Board of Directors. Dr. Manasse also serves as Professional Secretary of the Board of Pharmaceutical Practice of the International Pharmaceutical Federation (FIP). In 2006, he was appointed chair of the newly formed Board of Governors of the National Patient Safety Foundation (NSPF). In addition, Dr. Manasse has chaired the Sentinel Event Alert Advisory Group at the Joint Commission on Accreditation of Healthcare Organizations (JCAHO), and is a member of the Institute of Medicine of the National Academy of Sciences.
Dr. Manasse received his bachelor's degree in pharmacy from the University of Illinois at the Medical Center, a master's degree in educational psychology from Loyola University of Chicago, and a doctorate degree in pharmacy administration from the University of Minnesota.
Most recently, Dr. Manasse has been named the recipient of the American Society of Health-System Pharmacists 2007 Harvey A.K. Whitney Lecture Award.

Philip J. Schneider, Treasurer

Philip J. Schneider, B.S., Pharm.D., is Director of Pharmacy, Olathe Medical Center, Olathe, Kansas. He earned his bachelor's and doctorate degrees from the University of Iowa and completed a residency in Adult Internal Medicine from the Medical University of South Carolina. In his nearly 20 years with the Olathe Health System, he has led many programmatic and technological initiatives, which have resulted in a robust practice model and cost-effective drug utilization.
Dr. Schneider has served ASHP in a variety of capacities, including: member, Board of Directors (2004-2007); Chair, Council on Organizational Affairs; member, Committee on Nominations; and ASHP delegate for approximately 8 years. He is a past Treasurer and Presidential officer of the Kansas Society of Health-System Pharmacists (KSHP). He was the recipient of KSHP’s Harold Godwin lecture award for outstanding achievement in hospital pharmacy (1994) and the KSHP Pharmacist of the Year (1997). |

Stephen J. Allen, R.Ph., M.S., FASHP, has served as the ASHP Foundation's executive vice president since 2000. He has extensive health-system pharmacy management experience, having served as director of pharmacy at Children's Hospital National Medical Center, Kaiser Permanente, and Georgetown University Hospital. Mr. Allen received his bachelor's degree in pharmacy from the University of Rhode Island, and his master's degree in hospital pharmacy at the University of Maryland, Baltimore, where he also completed a residency in hospital pharmacy practice. |

Barbara Balik, R.N., M.S., Ed.D., is an independent healthcare consultant focusing on quality and safety. Previously, she worked at Allina Hospitals and Clinics in St. Paul, MN, filling three roles: Executive Vice President of Safety and Quality Systems, the System Patient Safety Officer and the Chief Quality Officer. Ms. Balik completed her doctorate degree in education at the University of St. Thomas in St. Paul, and earned her master's degree in family nursing from the University of Minnesota. Her bachelor's degree in nursing came from Marycrest College in Davenport, IA.
Ms. Balik serves on the National Patient Safety Foundation's Board of Governors and has also been a member of the selection panel for the ASHP Foundation's Award for Excellence in Medication-Use Safety for 3 years.
As a leader and nationally recognized expert in safety and quality systems, Ms. Balik has presented numerous times on these topics. She is passionate about building leadership skills and systems to transform health care. |

Michael S. Flagstad, Director at Large

Michael S. Flagstad, M.S., R.Ph., is founder and principal of Visante, Inc., a Minneapolis-based company that creates new safety nets for patients and providers in the delivery of care, including process improvement, guideline adoption and medication adherence.
Mr. Flagstad's career includes more than 15 years of experience in hospitals and academic medical centers in administrative, faculty and clinical practice roles. He has served as associate director of pharmacy at Shands Hospital at the University of Florida, clinical coordinator at Ingalls Memorial Hospital in Harvey, Illinois, and as a clinical pharmacist at Mercy Hospital Center in Des Moines, Iowa. Mr. Flagstad has also held pharmacy faculty appointments at the University of Florida, Purdue University and Drake University.
Mr. Flagstad has also served as senior Vice Preisdent and Chief Clinical Officer for Express Scripts/ValueRx. He has authored or coauthored more than 40 publications and presented more than 200 invited presentations and workshops at professional conferences and seminars. He is the 2000 recipient of the Winston J. Durant Lectureship Award from the University of Wisconsin Hospitals and Clinics. In 1999, Mr. Flagstad was an Honorary Fellow at the Center for Health-System Research and Analysis at the University of Wisconsin-Madison.
Mr. Flagstad earned his bachelor of science degree in pharmacy and master of science degree in hospital pharmacy from the University of Wisconsin-Madison, and completed his residency at the University of Wisconsin Hospitals and Clinics in Madison. |

Dallas J. Howe, Director at Large
Saskatoon, Saskatchewan, Canada
Dallas J. Howe is the Chair of the Board of Potash Corporation of Saskatchewan, the worlds' largest fertilizer enterprise. He is also a director of Viterra and chairs the Compensation Committee of Viterra, a Canadian agribusiness built on the foundation of Saskatchewan Wheat Pool, Inc. and Agricore United.
Presently, Mr. Howe is also CEO and owner of DSTC, a technology investment company, Chair of the Board of Advanced Data Systems, a business technology company and on the Board of the C.D. Howe Institute.
During Mr. Howe's business career, he pioneered, developed and commercialized applications software in the computer industry, as a founder and as CEO of several technology companies, including BDM Information Systems, a provider of health care software to major hospitals in the United States and Canada. BDM was in the top 50 Canadian software companies when General Electric Healthcare acquired BDM in 2002.
Mr. Howe has also served on the Board of Governors of the University of Saskatchewan and served as Chair of the Board.
Mr. Howe has also served on the Board of Saskatoon City Hospital Foundation and was a founding director of the Saskatoon Regional Economic Authority.
Mr. Howe has also served on and chaired Corporate Governance and Nominating, Audit and Compensation committees in the private, public and not-for-profit sectors. As well, he has served in the role of Board advisor to a publicly traded company.
Mr. Howe has honors bachelor's and master's degrees in mathematics from the University of Saskatchewan. He also attended the University of Toronto for graduate studies in Computer Science. Mr. Howe is the recipient of a KPMG Entrepreneur of the Year award. In 2009 Mr. Howe was made an ICD Fellow by the Institute of Corporate Directors.
Mr. Howe and his family run a fourth-generation family farm that has been operated continuously for more than 125 years. |

Lynnae Mahaney, Director at Large
Lynnae M. Mahaney, M.B.A, FASHP, is the Chief of Pharmacy Services at the VA Medical Center in Madison, Wisconsin, where she is responsible for the comprehensive pharmacy services for more than 35,000 veterans and several residency programs. Lynnae’s practice focus is medication safety, including medication reconciliation and technology integration. Her career has been devoted to the advancement of pharmacy services in hospital and health systems within the VA, community hospitals, medical clinics and home care. She received her bachelor of science degree from the University of Wisconsin (UW), Madison and master's degree in business administration from the UW-Milwaukee, Executive Program.
Mahaney has served ASHP on the Board of Directors, Councils on Legal and Public Affairs (Chair) and Organizational Affairs, Nominations Committee, Task Force on Pharmacy’s Changing Demographics, Foundation Development Committee, and as a Wisconsin delegate. She has been a board member of the Pharmacy Society of Wisconsin (PSW) and named PSW 2004 Pharmacist of the Year.

Bruce E. Scott, Director at Large

Brude E. Scott is currently Senior Vice President and Chief Pharmacist of Medco Health Solutions. Prior to joining Medco in September 2008, Bruce has held various senior leadership roles, including Chief Operating Officer of McKesson Medication Management, a subsidiary of McKesson which provides pharmacy management services to hospitals and vice president positions in Allina Hospitals and Clinics in Minnesota. He has also held multiple pharmacy practice leadership roles, including Director of Pharmacy at United Hospital in St. Paul, Minnesota.
Mr. Scott earned his undergraduate degree in pharmacy from the University of Wisconsin School of Pharmacy. (He is a proud Badger!) He also earned his master’s degree in pharmacy administration from the University of Kansas.
Mr. Scott is passionate about the practice pf pharmacy, which is demonstrated through his publications and service in pharmacy organizations. He has served as president of both the Minnesota Society of Health-System Pharmacists and the American Society of Health-System Pharmacists.
Mr. Scott also gives of his time through service on a number of boards, including The Ronald McDonald House Charities, the Board of Advisors of the University of Wisconsin, and the Board of Advisors for the University of Minnesota.

Gordon S. Willcox, Director at Large

Gordon S. Willcox, M.S., R.Ph., is Vice President of Account Management for Roche Pharmaceuticals, which is headquartered in Nutley, New Jersey. Mr. Willcox has responsibility for large, organized managed care, PBM, government, hospital, chain drug stores, group purchasing organizations, specialty pharmacy and pharmaceutical distribution customers. He has served in this position since 2000.
Prior to rejoining Roche, Mr. Willcox was Vice President of Sales for Home Medical of America, a large national multi-service home care company. He held several senior-level sales and operations positions in Nations Healthcare/Chartwell Home Therapies. Nations Healthcare was acquired by Home Medical of America in 1999.
Roche previously employed Mr. Willcox from 1993 – 1995 as President of Roche Professional Service Centers, a home infusion therapy Roche subsidiary. Nations Healthcare acquired the company in 1995.
Prior to joining Roche, Mr. Willcox co-founded and was President of American Cardiology Care in Baltimore, Maryland. This company pioneered congestive heart failure disease management. Mr. Willcox sold the company to Critical Care America in 1993. He held numerous executive positions with Critical Care America, a national home infusion therapy provider from 1986 - 1993.
Mr. Willcox began his career as a hospital pharmacist in Rhode Island. He holds a bachelor of science degree in pharmacy from the University of Rhode Island and a master of science degree in radiopharmaceutical Science from Northeastern University.
