In this compelling portrait of a pharmacist-led initiative to improve medication use within kidney transplantation, attendees discovered ways to implement safety improvements in their own institutions that will improve medication safety for all hospitalized patients.

Learning Objectives:
• List initiatives that pharmacists undertook to improve medication use within kidney transplantation.
• Describe challenges that occurred during the implementation of these initiatives and how resolution of these challenges led to improved patient outcomes.
• Explain how these initiatives can be incorporated into pharmacists daily activities to improve medication use safety across all hospitalized patients.
