Kate Farthing

Kate Farthing is a clinical coordinator at Legacy Salmon Creek Medical Center in Vancouver, Washington.  Legacy Salmon Creek is part of Legacy Health, an eight hospital system in the Portland, Oregon, metropolitan area. Kate is the liaison between pharmacy and information systems, and is active in the Legacy PGY1 residency program as a preceptor and member of the residency advisory committee. Prior to joining Legacy in June 2010, Kate spent 16 years at Oregon Health and Sciences University Hospitals and Clinics in drug information/drug policy with resident and student teaching and training responsibilities, including residency program director for the PGY1 program and a PGY2 drug information specialty residency program.

Kate received her BS of pharmacy and PharmD degrees from the University of Kansas and completed a specialty residency in drug information at the University of Kansas Medical Center. Kate completed the Pharmacy Leadership Academy program in 2009.

She is active in state and national organizations, including serving as a delegate for the state of Oregon at the ASHP House of Delegates, the public relations liaison with other pharmacy societies in the state and serving as the cochair of the OSHP Annual Seminar. Kate is a past chair of the ASHP Commission on Credentialing and has participated in the ASHP Public Relations Action Network for many years.
