The ASHP Foundation Diabetes Patient Care Traineeship is a certificate program that provides pharmacy continuing education (CE) credit for both the self-study and the experiential components. Participants must successfully complete the online self-study test before they can participate in the experiential component to earn full credit for this certificate program.
Participants will be able to print their CE Statement for the self-study component immediately after taking the online test. For the experiential component, participants must complete a CE request form at the end of the experiential training and ASHP will mail a validated CE statement within 4 to 6 weeks.

The ASHP Research and Education Foundation will award each participant a certificate for successfully completing the self-study and experiential components as well as the post-traineeship assignments.
