To request a transcript and an invoice for fees paid to the ASHP Foundation, students enrolled in the Academy or a Capstone Course must follow the steps below:

1.  Contact your employer's human resources department and complete any required paperwork.

2. Complete a "Permission to Release Educational Records to Employer" form.

3.  Upon receipt of the release form, GlobalHealth Education (GHE) will contact the ASHP Foundation to request proof of your Academy or Capstone Course payment. GHE will then send a copy of your transcript and proof of payment to the contact you have indicated on the release form.

4. If you do not need a transcript and want only an invoice, you may request an invoice by contacting the ASHP Foundation at foundation@ashp.org.
