Prerequisites for the Antithrombotic Traineeship

1.  Qualifications of an Applicant: An applicant must be a licensed pharmacist in any state in the U.S. and be a graduate of an ACPE-accredited college or school of pharmacy. The applicant must have completed an ASHP-accredited residency or have 2 years of experience in clinical practice. Previous educational preparation and clinical practice or ambulatory care experience are prerequisites for the training. Trainees selected for the traineeship should have solid didactic preparation in antithrombotics.

Applicants must have a command of basic knowledge areas as they relate to patients with a thromboembolic condition including pathophysiology, clinical pharmacology and therapeutics, clinical laboratory data interpretation, clinical pharmacokinetics, and medical terminology and abbreviations. In addition, applicants should have experience with writing pharmaceutical care plans, conducting patient interviews, and delivering educational programs to other members of the health care team.

Pharmacists selected for the traineeship program will need to provide proof of licensure, professional liability insurance (either held by you or your institution), current health status (updated TB testing) and HIPAA training.

2.  Applicant’s Institution:  The site of practice must be a health-care system in the U.S. with resources for the provision of services for patients with thromboembolic conditions. The chief executive officer of the system, and the physician who is responsible for the management or prevention of patients with thromboembolic conditions must provide documentation of their commitment to the establishment of a pharmaceutical service for patients with thromboembolic conditions. 

3.  Complete the Improving Patient Outcomes in Thromboembolic Disorders Self-Assessment CE Tool:  Pharmacists interested in applying for the Antithrombotic Pharmacotherapy Traineeship need to complete the self-assessment tool associated with the “Improving Patient Outcomes in Thromboembolic Disorders” and complete the recommended level of programming offered through this portal. The goal is to assist potential trainees in preparing for the experiential training program. This tool is designed to assist applicants in determining their educational needs. The self-assessment CE tool is brief; the estimated time to complete this tool is about is 5-10 minutes. The tool can be found at  http://www.improvingpatientoutcomes.com/

4.  Sign the Experiential Training Agreement:  Pharmacists selected to participant will be challenged to set specific and measurable goals for managing the antithrombotic therapies for patients at their practice site. After participating in the experiential activity trainees will be required to complete a post-training analysis that demonstrates the impact of the traineeship on antithrombotic pharmacotherapy services at their site.
