The objectives of this study are to quantify the association between nonadherence with INR monitoring and the occurrence of bleeding, thromboembolic amd fatal warfarin-related adverse events and to identify patient-specific characteristics that are associated with INR monitoring nonadherence.

Improving Pharmacist-Led Electronic Medication Reconciliation

The objectives of this study are to compare medication history obtained for patients upon admission by three mechanisms - patient interview, electronic medical records (EMRs) and discharge records, and pharmacy claims data; to evaluate the validity of aggregated medication sources to predict medication non-adherence; and to develop and evaluate a prototypical user interface for displaying medication history and adherence information to pharmacists.
